This form is used to request a change in program of study, e.g., a change in degree, major, area of emphasis or concentration. This form cannot be used to change from non-degree to degree-seeking. Changes in mailing addresses, email addresses or phone numbers can be made through MyDSCC rather than using this form.
Faculty may wish to utilize this form for a student who is not present at the first class meeting. A copy of the completed form - signed by the instructor and the student - should be sent to the Office of Records.
Use this form to officially withdraw from DSCC.
This form is used to: - Request to add a course section after the Last Day to Register or Add; special permission is required. - Request to drop a course section after the Last Day to Drop or Withdraw; special permission is required. - Request to Audit a course section up to the Last Day to Audit. The completed form - with all signatures - should be submitted to the Office of Records.
The Course Overload Form is used to request permission to take more than 19 hours during the fall or spring semester or more than 14 hours during the summer term.
The Application for Credit by Examination is used to initiate the process of taking an examination in a specific DSCC course in anticipation of receiving credit for the course based on your current knowledge of the course content.
The Enrollment Verification form authorizes DSCC to release information about your dates of attendance, enrollment status and GPA to the requested party.

The CTE Articulation Agreement is used to gain credit hours for high school courses which articulate into DSCC college-level courses. Individuals who completed Career Technical Education (CTE) courses in high schools may be eligible to receive DSCC credit. Refer to this document for more information.
The Tennessee Early Childhood Program Administrator Credential (TECPAC) is the recognition awarded to early childhood administrators who have demonstrated the specific competencies for effective leadership and management through academics, experiences, and portfolio assessment.
