This form is used to request a change in program of study, e.g., a change in degree, major, area of emphasis or concentration. This form cannot be used to change from non-degree to degree-seeking. Changes in mailing addresses, email addresses or phone numbers can be made through MyDSCC rather than using this form.
Faculty may wish to utilize this form for a student who is not present at the first class meeting. A copy of the completed form - signed by the instructor and the student - should be sent to the Office of Records.
Use this form to officially withdraw from DSCC.
